Skilled Nursing

– To ensure continuity of care NHC will in most circumstances have a Registered Nurse admit the patient and will be seeing the patient there after. At NHC, we do not believe in sending different care teams every week to ensure high quality and also to maintain and build a relationship with the patient and their family by the same RN going to see the patient almost every time.

Physical Therapist

– Our Physical therapist will follow the individualized plan of care to maximize speedy recovery from an acute condition and/or have therapy for managing chronic conditions.

Occupational Therapy

– Our team of occupational therapists work individually with their patients to develop recover or maintain activities of daily living to keep our patients in the comfort of their own homes.

Speech Pathology

– Our Speech therapists access for on speech, language, cognitive communication and oral/feeding/swallowing skills to identify the type’s problems with the patient to then try to individualize the care plan to best treat the problem.

Medical Social Worker

– Our Social workers conduct an assessment on patients to determine the needs and identify limitations, barriers or safety issues that might impede recovery, educate patients and families regarding community resources and support, and assist with long range planning.
