Fall Safety and Recovery

Today falls in the home is the number one reason in the United States of America for senior citizens requiring moving into nursing homes. At NHC, every patient at the time of admission into the program receives a complete assessment of muscle strength, flexibility, balance, gait, layout of the house and any hazardous conditions in the home that could lead to a fall. Once assessed, the Care Team will in the patients individualized care plan will add specific action plans to reduce the risk of falls in the home.

Pulmonary

Common pulmonary conditions include but are not limited to Congestive Obstructive Pulmonary Disease, Pneumonia, and Pulmonary edema. Pulmonary conditions often cause lowered immune system making the patient susceptible to infections. Decreased oxygen flow and reduced lung capacity often causes shortness of breath with minimal exertion. To help our patients the Registered nurse will work with the physician and focus on instructing and educating on oxygen use, exercises to increase lung volume, proper medication management to prevent exacerbation of a disease. The physical therapist will develop a care plan to build strength and endurance. Occupational therapy will build upper body strength to make sure that the patient can carry out activities of daily living. Having a multidisciplinary approach and an individualized care plan for every patient will help optimize quality of life and increase greatly the chances of our patients to continue living in the comfort of their own homes.

Cardiac

Patients with Cardiac disorders ranging from Congestive heart failure to Hypertension, Coronary artery disease etc., need to be assessed individually to determine the best care plan to keep our patients out of urgent care and hospitals. Instructing and educating the patient and families on the signs and symptoms of the disease process, reviewing and reconciling medication every time the Registered Nurse comes to see the patient, ensuring that vital signs are always recorded and transferred in real time to the care team along with following the care plan set in place by the physician after consultation of the Care team will ensure speedy relief and recovery.

The main emphasis in Stroke Recovery is to regain functional mobility and decrease potential complications during recovery. Educating patients and families to watch for early warning signs Along with ensuring safety and fall preventative measures in the house will prevent hospitalization due to accidents.
